We may charge additional fees for the following services.

No-show fee, Well Visit	$50	24-hour notice must be given for cancelled appointment
Physical forms	$5	Children will be given a universal healthcare form at each well visit. If you misplace this and need a new one, there will be a $5 charge
other forms	$10	There is a $10 fee for all non standard forms (camp, college, etc). please allow 1 week for all forms to be completed.
